The Head-to-Head challenge of Miss World 2018 involved 118 contestants first grouped into 12 for the initial round. The winners advanced to a second round judged by former Miss World titleholders, resulting in 10 pairs competing. Ultimately, 10 winners progressed to the Top 30 of the competition.
